위축성 위염, 조직검사 없이도 선별 가능 시대 '개막'

한국건강검진학회가 지난 23일 소공동 롯데호텔에서 연  춘계학술대회에서 위암 발생 고위험군 조기 선별을 위한 진단 방법으로 혈청 바이오마커 기반 검사‘GastroPanel’이 학계 주목을 받았다.

대한상부위장관·헬리코박터학회(2022) 위염 진료지침에 따르면, 위암 예방을 위해 위축성 위염 및 장상피화생을 진단하고 이를 위험 단계에 따라 추적 관찰할 필요가 있다.

현재까지는 내시경 및 생검을 통한 조직학적 확인이 필수였으나, 조직검사 없이도 비침습적으로 위 점막 상태를 평가할 수 있는 대안으로 GastroPanel이 부상하고 있다.

정부는 위암을 ‘예방 가능한 암(preventable cancer)’으로 분류하고, 주요 원인인 헬리코박터 파일로리(H. pylori)의 조기 치료 확대를 위해 국가암검진사업 내 선별검사 도입을 검토 중이다.

2020년 국립암센터에 따르면, 헬리코박터로 인한 위암 발생 기여율은 남성 34.5%, 여성 33.2%에 달하지만 현재 국내 헬리코박터 감염률은 43%로 여전히 높고, 제균 치료율은 23%에 불과한 실정이다.

GastroPanel은 한국인을 대상으로 기준치(reference range)를 별도 설정하여, 국내 임상 현장에서 개인별 위 상태에 맞는 진단 및 맞춤형 치료 전략 수립이 가능하다.

세계적인 위장관 병리학자이자 GastroPanel 개발 자문을 맡았던 Dr. Kari Sipponen (전 헬싱키대학교 병리학과장)은 이날 "조직검사 없이 육안 소견에 의존하는 한국 내시경 환경에서는, GastroPanel 검사가 위축성 위염을 선별하고 추적 관리하는 데 매우 유용한 도구가 될 것”이라고 강조했다.

At the 2024 Spring Conference of the Korean Society of Health Screening (KSHS), held on March 23 at Lotte Hotel in Sogong-dong, the serum biomarker-based test GastroPanel received significant attention from the academic community as a promising diagnostic method for early identification of high-risk groups for gastric cancer.

According to the 2022 clinical guidelines on gastritis issued by the Korean College of Helicobacter and Upper Gastrointestinal Research, the prevention of gastric cancer requires the diagnosis and monitoring of atrophic gastritis and intestinal metaplasia, based on their severity and progression.

Traditionally, endoscopy and biopsy have been the standard approaches for evaluating these conditions. However, GastroPanel is emerging as a non-invasive alternative that can assess the condition of the gastric mucosa without the need for a tissue biopsy.

The Korean government classifies gastric cancer as a “preventable cancer” and is currently reviewing the inclusion of H. pylori screening in the National Cancer Screening Program as part of its efforts to promote early eradication of the bacteria—a leading cause of gastric cancer.

According to the National Cancer Center in 2020, H. pylori infection contributes to 34.5% of gastric cancer cases in men and 33.2% in women. Despite this, the national infection rate remains high at 43%, with an eradication rate of only 23%, underscoring the need for more proactive measures.

Tailored specifically to the Korean population, GastroPanel features locally validated reference ranges, enabling clinicians to assess individual gastric health more accurately and develop personalized treatment strategies.

Dr. Kari Sipponen, a renowned gastrointestinal pathologist and former Head of Pathology at the University of Helsinki, who served as a scientific advisor for the development of GastroPanel, stated during the event:
“In Korea, where endoscopic diagnosis often relies on visual assessment without biopsy, GastroPanel provides a highly effective tool for screening and monitoring atrophic gastritis.”
